New patient information

Thank you for choosing Elite Foot & Ankle specialists for your care. We thrive to provide elite care to everybody without discrimination of any kind. We mobilize all of our resources to help all our patients knowing that the care we provide is the care we would expect for ourselves and our friends and family.

Scheduling and cancellations

To schedule an appointment please call during normal business hour at (239)689-5130. If for any reason the staff is not available to receive your call please leave a brief message including your phone number, and your call will be returned. Or simply click on “request an appointment” to send us an email and someone will contact you.

If for any reason you cannot make it to your appointment, please give us 24-hour notice so the slot can be open to other patients.

What to bring for your appointment

Bring a picture ID, your insurance Cards, and if possible, fill forms obtained from the website in the new patient section.

When to arrive

For new patients, please arrive 15 minutes in advance to allow adequate time for you to complete any necessary paperwork. If you would like to complete the paperwork prior to your visit, please click here to download our patient forms.
